Mikel Anne Hofmann, MD

Dr. Hofmann has been serving patients in the Washington, D.C. metropolitan area since 2009, though she is originally from Albany, Georgia. She earned both her B.S. and M.D. from The Florida State University and completed her residency training at the Georgetown University/Providence Hospital Family Medicine Residency Program in 2012.

A board-certified family physician, Dr. Hofmann’s clinical interests include adult and pediatric care, women’s health, and long-term continuity of care. Before founding CuraCapitol, she served as the Medical Director of a local clinic dedicated to caring for underserved communities. She also teaches as a Family Medicine preceptor for Georgetown University School of Medicine, helping train the next generation of physicians.

Dr. Hofmann has been recognized as a Washingtonian Magazine “Top Doctor” in both 2024 and 2025. Outside of medicine, she enjoys running, spending time with her family, volunteering, and engaging in advocacy work.

 

Sarah g. candler, MD, MPh

Dr. Candler brings a wealth of experience in primary care, academic medicine, and health policy to our practice. She is the former Practice Medical Director at Iora Primary Care in Houston, Texas, where she led a value-based care model focused exclusively on Medicare patients through a team-based, relationship-centered approach to senior care.

Dr. Candler earned both her MD and Master of Public Health in Health Policy and Management at Emory University, before completing her internal medicine residency in the primary care track at Tulane University in New Orleans. She went on to practice as a general academic internist at the Michael E. DeBakey VA Medical Center in Houston—one of the nation’s highest-volume and highest-complexity VA hospitals—and served as an Assistant Professor of Medicine at Baylor College of Medicine.

In addition to her clinical work, Dr. Candler has been deeply involved in national health leadership and education. She has chaired the American College of Physicians’ Council of Resident and Fellow Members, currently serves as an ACP delegate to the AMA, where she serves on the Governing Council of the Specialty and Service Society. She has written curricula on health policy, advocacy, economics, and the impact of social determinants of health.

Beyond the clinic and classroom, Dr. Candler serves on the Advisory Board of Internal Medicine News and was a founding editor for Fresh Look, the Annals of Internal Medicine blog.

Now based in Washington, D.C., Dr. Candler lives with her husband and children. We are grateful for the depth of knowledge, compassion, and leadership she brings to our team.

Crsytal smalls-gonzalez, pa-c

Ms. Smalls-Gonzalez brings more than 30 years of experience in primary care to our practice. A graduate cum laude of the Howard University Physician Assistant Program, she is nationally certified by the National Commission on Certification of Physician Assistants (NCCPA).

Throughout her career, Ms. Smalls-Gonzalez has been dedicated to building strong relationships with patients and providing comprehensive, compassionate care. She is especially passionate about supporting patients in achieving long-term wellness and guiding them through every stage of life.

She resides in Maryland with her husband, and outside of medicine enjoys traveling and giving back through her service to patients and community.
